Background:

Acute Retinal Necrosis Syndrome (ARNS) is a severe, rapidly progressive necrotizing retinitis.
Mono-ophthalmic status presents unique challenges in managing sight-threatening conditions.

Observation:

A 29-year-old mono-ophthalmic patient presented with ARNS.
The condition exhibited rapid progression despite initial medical management.
Complications included complex retinal detachment, recurrent intraocular inflammation, and complicated cataract.

Findings:

Multiple surgical interventions were required to address the ARNS complications.
A successful and stable functional visual result was ultimately achieved.
This case highlights the potential for visual recovery even in severe ARNS cases.

Implications:

Effective management of ARNS in mono-ophthalmic patients requires a multi-faceted surgical approach.
Early and aggressive intervention may be crucial for preserving vision.
Further research into ARNS pathogenesis and treatment strategies is warranted.
