Background:

  • Posterior fossa lymphoma is a rare but aggressive malignancy.
  • Diagnosis can be challenging due to nonspecific initial symptoms.

Observation:

  • Six patients with posterior fossa lymphoma were retrospectively reviewed.
  • Common initial symptoms included vertigo (100%) and ataxia (67%).
  • Audiometry revealed sensorineural hearing loss, and electronystagmography (ENG) showed central nystagmus signs.

Findings:

  • Magnetic resonance imaging (MRI) demonstrated enhanced mass lesions in the cerebellum and/or brainstem.
  • Cerebrospinal fluid (CSF) cytology or histopathology confirmed lymphoma in all cases.
  • Treatment involved irradiation and chemotherapy, with varied outcomes.

Implications:

  • Persistent vertigo and ataxia with central ENG signs warrant consideration of posterior fossa lymphoma.
  • Early diagnostic imaging (MRI) and CSF analysis are vital.
  • Increased awareness can improve timely diagnosis and management of this rare condition.
