Summary
Oral squamous cell carcinomas (SCCs) can invade bone. This study found that parathyroid hormone-related protein (PTHrP) expression in oral SCC may indicate bone involvement and stimulate bone resorption.
Area of Science:
- Oncology
- Oral Pathology
- Molecular Biology
Background:
- Matrix metalloproteinases (MMPs) are implicated in carcinoma metastasis to soft tissues.
- Osteoclastic bone resorption is crucial in bone metastasis of various cancers, but its mechanism in oral SCC is unclear.
- Bone provides a supportive microenvironment for cancer cell growth due to stored cytokines and growth factors.
Purpose of the Study:
- To investigate the molecular mechanisms underlying bone involvement in oral squamous cell carcinomas (SCCs).
- To identify specific factors expressed by oral SCC cells that may contribute to bone resorption and metastasis.
- To explore the potential role of parathyroid hormone-related protein (PTHrP) in oral SCC bone invasion.
Main Methods:
- Analysis of gene expression (mRNA) in the bone-invasive oral cancer cell line BHY, including TGF-beta, IL-1beta, IL-8, PTHrP, VEGF, GM-CSF, IL-6, and TNF-alpha.
- Incubation of osteoblasts with BHY-conditioned medium to assess effects on IL-6 and RANKL/ODF mRNA expression.
- Detection of PTHrP mRNA in tumor samples from patients with oral carcinomas and bone involvement.
Main Results:
- The BHY cell line expressed detectable levels of TGF-beta, IL-1beta, IL-8, PTHrP, and VEGF mRNA.
- BHY-conditioned medium significantly upregulated IL-6 and RANKL/ODF mRNA in osteoblasts, suggesting indirect stimulation of osteoclastogenesis.
- PTHrP mRNA was detected in 7 out of 11 patients with oral carcinomas exhibiting infiltrative bone involvement.
Conclusions:
- Oral SCC cells express factors that can potentially stimulate osteoclastogenesis and bone resorption.
- Parathyroid hormone-related protein (PTHrP) expression in oral SCC may be an indicator of bone invasion.
- These findings elucidate potential mechanisms of bone involvement in oral SCC and highlight PTHrP as a potential biomarker.

Osteoclasts in Bone Remodeling
Osteoclasts are cells responsible for bone resorption and remodeling. They originate from hematopoietic progenitor cells present in the bone marrow. Numerous progenitor cells fuse to form multinucleated cells, each with 10-20 nuclei. A single osteoclast has a diameter of 150 to 200 µM. These cells have ruffled borders that break down the underlying bone tissue and release minerals such as calcium into the blood in bone resorption. Osteoclasts cling to bones with their ruffled edges during bone...
Bone Cells and Tissue
Bones contain a relatively small number of cells entrenched in a matrix of organic and inorganic components. Although bone cells compose only a small amount of the bone volume, they are crucial to its function. Four types of cells are found within the bone tissue— osteoblasts, osteocytes, osteogenic cells, and osteoclasts.
Osteoblasts and Osteocytes
The osteoblast is the bone cell responsible for forming new bone tissue. It is found in the growing portions of bone, including the periosteum and...

Bone Remodeling
Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.
