Sjögren syndrome (SS) rarely presents as the initial symptom of multiple myeloma (MM). Treating the multiple myeloma led to the remission of Sjögren syndrome symptoms in this case.

- Further investigation revealed IgA lambda multiple myeloma with diffuse lytic bone lesions.
- The patient experienced persistent thoracic pain, indicative of advanced disease.
Findings:
- The patient received VAD chemotherapy and bisphosphonate therapy for multiple myeloma.
- Following anti-myeloma treatment, the multiple myeloma regressed.
- Remarkably, Sjögren syndrome symptoms also resolved after MM treatment.

- This case highlights the unusual presentation of multiple myeloma as Sjögren syndrome.
- The findings suggest a potential shared aetiopathogenic pathway between SS and MM.
- Effective MM treatment may lead to remission of associated SS symptoms.
