Summary

Intravitreal triamcinolone acetonide (IVTA) injection improves B-scan ultrasonography visualization of vitreoretinal patterns in eyes with opaque media. This technique aids in planning pars plana vitrectomy (PPV) and assessing prognosis.

Background:

Opaque media in the eye presents challenges for visualizing posterior segment structures.
Accurate assessment of vitreoretinal patterns is crucial for surgical planning and prognosis.

Observation:

B-scan ultrasonography was used to examine three eyes with opaque media (one with pseudophakic cystoid macular edema, two with endophthalmitis) before and after intravitreal triamcinolone acetonide (IVTA) injection.
Two eyes with endophthalmitis underwent pars plana vitrectomy (PPV) on the same day as IVTA injection.

Findings:

Pre-injection B-scan showed relatively anechoic vitreous in all cases.
Post-IVTA injection, visualization of the vitreous and posterior hyaloid face (PHF) improved, revealing vitreoretinal adhesions.
Echographic findings correlated with intraoperative observations in the vitrectomized eyes.

Implications:

Triamcinolone-assisted ultrasonography enhances imaging of the PHF in eyes with opaque media.
This modality can be valuable for planning PPV and predicting outcomes in challenging cases.
Careful consideration of diagnostic benefits versus IVTA risks is advised for selected applications.
