Background:

  • Opaque media in the eye presents challenges for visualizing posterior segment structures.
  • Accurate assessment of vitreoretinal patterns is crucial for surgical planning and prognosis.

Observation:

  • B-scan ultrasonography was used to examine three eyes with opaque media (one with pseudophakic cystoid macular edema, two with endophthalmitis) before and after intravitreal triamcinolone acetonide (IVTA) injection.
  • Two eyes with endophthalmitis underwent pars plana vitrectomy (PPV) on the same day as IVTA injection.

Findings:

  • Pre-injection B-scan showed relatively anechoic vitreous in all cases.
  • Post-IVTA injection, visualization of the vitreous and posterior hyaloid face (PHF) improved, revealing vitreoretinal adhesions.
  • Echographic findings correlated with intraoperative observations in the vitrectomized eyes.

Implications:

  • Triamcinolone-assisted ultrasonography enhances imaging of the PHF in eyes with opaque media.
  • This modality can be valuable for planning PPV and predicting outcomes in challenging cases.
  • Careful consideration of diagnostic benefits versus IVTA risks is advised for selected applications.
