  • Re-challenge with oral and nasogastric thyroxine administration led to recurrent hypothyroidism, confirming apparent isolated levothyroxine malabsorption.

Implications:

  • Intramuscular thyroxine injections represent a viable alternative therapeutic strategy for patients with documented levothyroxine malabsorption.
  • This case highlights the importance of considering alternative drug delivery routes when standard oral administration fails.
