Summary

Pyoderma gangrenosum can indicate a serious underlying blood disorder transformation. Re-biopsy of non-healing ulcers is crucial in patients with myelodysplastic syndrome to detect myeloid sarcoma and guide treatment.

Background:

  • Pyoderma gangrenosum (PG) is a rare, destructive neutrophilic dermatosis.
  • Myelodysplastic syndromes (MDS) are a group of clonal hematopoietic stem cell disorders.
  • Transformation of MDS to acute myeloid leukemia (AML) is a critical clinical event.

Observation:

  • A patient with myelodysplastic syndrome presented with pyoderma gangrenosum.
  • The pyoderma gangrenosum ulcers showed secondary blastic infiltration.
  • This infiltration was indicative of transformation to myeloid sarcoma, a precursor to acute leukemia.

Findings:

  • The patient received intensive chemotherapy.
  • Complete remission of the hematological malignancy was achieved.
