Remote ischaemic pre-conditioning (RIPC) did not protect the left ventricle (LV) from dysfunction during coronary occlusion. This study found RIPC ineffective in preventing or improving LV ischaemic injury in patients with coronary disease.

Background:

  • Remote ischaemic pre-conditioning (RIPC) is known to reduce distant tissue injury from ischaemia and reperfusion.
  • The protective effects of RIPC on the left ventricle (LV) against ischaemic dysfunction and stunning remain incompletely understood.

Purpose of the Study:

  • To test the hypothesis that RIPC protects the left ventricle (LV) from ischaemic dysfunction and stunning.
  • To evaluate the efficacy of RIPC in mitigating LV injury during coronary occlusions.

Main Methods:

  • Forty-two patients with single-vessel coronary disease and normal LV function were recruited.
  • LV function was assessed using conductance catheter measurements and dobutamine stress echocardiography with tissue Doppler analysis.

  • RIPC was induced by inflating a blood pressure cuff around the upper arm for 5-minute intervals.
  • Main Results:

    • RIPC did not reduce the degree of LV dysfunction during coronary balloon occlusion (P = 0.15).
    • Evidence of cumulative LV dysfunction was observed despite RIPC (P = 0.03).
    • RIPC did not improve contractile recovery during reperfusion (P = 0.88) or affect LV function assessed by tissue Doppler (P = 0.43).

    Conclusions:

    • Remote ischaemic pre-conditioning does not attenuate ischaemic left ventricle dysfunction in humans.
    • RIPC demonstrated a neutral effect on LV function in patients undergoing coronary interventions.
