A 13-year-old male presented with dysuria and post-void dribbling.
Digital rectal examination revealed a palpable cystic mass.
Pelvic ultrasound and MRI confirmed a large cystic lesion communicating with the posterior urethra.

Findings:

Surgical resection via a posterior sagittal approach was successfully performed.
The patient experienced complete symptom resolution post-operatively.

Implications:

This case highlights the importance of considering prostatic utricle cysts in pediatric patients with urinary complaints.
