Summary

This study demonstrates successful bi-directional conversion between openEHR archetypes and proprietary EHR templates, enabling interoperability. Legacy EHR systems can incrementally adopt archetype support for improved clinical data sharing.

Background:

Electronic Health Record (EHR) data exchange faces challenges due to diverse proprietary models.
openEHR and CEN/ISO have developed archetype-based methodologies for EHR communication.
Limited experience exists with deploying archetypes in large-scale EHR systems.

Purpose of the Study:

To investigate the feasibility of converting proprietary EHR content models to openEHR archetypes.
To assess the inverse conversion of openEHR archetypes to a proprietary EHR format.
To evaluate the semantic consistency and completeness of converted models.

Main Methods:

Utilized openEHR EHR Reference Model (RM) and Archetype Model (AM) specifications.
Analyzed and compared the Cambio COSMIC template model with openEHR RM/AM.
Developed prototype software for automated bi-directional conversion based on a semantic mapping.

Main Results:

Achieved automated bi-directional conversion between openEHR archetypes and COSMIC templates.
Successfully imported openEHR archetypes into COSMIC, preserving constraints.
Converted COSMIC templates to openEHR archetype format, retaining structural and semantic definitions.

Conclusions:

openEHR RM/AM are sufficiently expressive for existing proprietary EHR content models.
Legacy EHR content models can be automatically converted to archetypes for knowledge sharing.
Incremental adoption of archetype support offers a migration path towards standards-based interoperability.
