Background:

  • CD16b is an Fc gamma receptor on neutrophils, crucial for immune responses.
  • Neutrophil alloantigens and glycosyl phosphatidylinositol (GPI) linkage are vital for neutrophil function.

Observation:

  • Two female patients, aged 41 and 15, presented with distinct hematological conditions.
  • Flow cytometry revealed absent CD16b expression (null phenotype) in both patients.
  • No deficiencies in other neutrophil alloantigens or GPI linkage defects were observed.

Findings:

  • The CD16b null phenotype was confirmed in both patients without associated GPI anchor defects.
  • Clinical symptoms were not solely attributable to CD16b absence, suggesting compensatory mechanisms by CD32 and CD64.

Implications:

  • CD16b deficiency, occurring in <1% of Caucasians, necessitates consideration in transfusion medicine to prevent alloimmunization.
  • Awareness of this rare phenotype is essential for managing potential transient neonatal immune neutropenia in offspring of affected mothers.
