Background:

Pulmonary carcinosarcoma is a rare, aggressive lung malignancy.
It is characterized by a mixture of carcinoma and sarcoma components.

Observation:

A unique case presented with cryptic clinical features, diagnosed post-mortem as stage IV lung malignancy.
Histopathology revealed a carcinosarcoma composed of adenocarcinoma and osteosarcoma.

Findings:

The tumor metastasized to the heart, lymph nodes, and both adrenal glands.
This represents the first reported instance of this subtype with cardiac metastasis.
