Summary

Prostate cancer rarely metastasizes to the optic nerve, causing vision loss. Early treatment with methylprednisolone can restore vision in these rare cases.

Background:

  • Optic nerve infiltration by tumors is more commonly primary than secondary.
  • Metastases to the optic nerve are rare, with prostatic carcinoma being exceptionally infrequent.

Observation:

  • A 66-year-old man experienced vision loss due to optic nerve metastasis two years post-prostate surgery.
  • Initial examination revealed papillary edema and optic nerve functional impairment; later examination showed signs of retrobulbar optic neuropathy.

Findings:

  • The patient's prostate carcinoma metastasized to the optic nerve, leading to severe vision loss.
  • Systemic methylprednisolone therapy resulted in significant recovery of visual function.

Implications:

  • This case highlights the rare occurrence of prostate cancer metastasis to the optic nerve.
  • Prompt diagnosis and systemic therapy can lead to favorable visual outcomes in such rare presentations.
