Summary
Extracranial carotid artery aneurysms are rare vascular conditions. Surgical intervention is often necessary to prevent severe complications and high mortality rates associated with these aneurysms.

Background:
- Extracranial carotid artery aneurysms are uncommon, representing a small fraction of all arterial aneurysms.
- Etiologies include atherosclerosis, fibromuscular dysplasia, trauma, infection, and congenital defects, with atherosclerosis being the most prevalent cause.
- These aneurysms most commonly occur at the common carotid artery bifurcation and proximal internal carotid artery (ICA).
Observation:
- Aneurysms typically present as a pulsatile parapharyngeal mass.
- They can lead to complications such as embolization, neurovascular compression, rupture, and ischemic events.
- Saccular aneurysms are common in the middle and distal ICA, while fusiform aneurysms occur at the bifurcation.
Findings:
- Atherosclerosis accounts for a significant majority (46-70%) of carotid artery aneurysms.
- Conservative management of extracranial internal carotid artery (ICA) aneurysms carries a high mortality rate of approximately 71%.
- Surgical treatment is generally indicated to avoid catastrophic outcomes.
