Philosophical, clinical, and bioethical models do not fully explain how specific beliefs derail rational deliberation.

Observation:

Traumatic life circumstances can lead to demoralization and catastrophic thinking.
Emotionally grounded beliefs, particularly those unresponsive to evidence, can systematically block deliberation.
Individuals with intact cognition may struggle to appreciate future quality of life during crises.

Findings:

A significant gap exists in understanding how emotionally grounded beliefs derail decision-making capacity.
Capacity-derailing reactions can occur even in individuals meeting standard medico-legal criteria.
Cognitive intactness does not preclude the inability to weigh future quality of life due to emotional distress.

Implications:

Existing assessments of cognition may be insufficient for determining decisional rights in medical and legal contexts.
Further research is needed to understand the mechanisms by which emotional beliefs impact decision-making.
Identifying the influence of emotionally grounded beliefs is essential for determining caregiver intervention obligations.
