Background:

Autoimmune pancreatitis (AIP) is a distinct subtype of chronic pancreatitis, predominantly affecting males.
It can manifest as obstructive jaundice or abdominal pain and may correlate with other autoimmune conditions and serological markers.
Radiological findings include pancreatic enlargement with ductal stenosis or a focal mass, posing diagnostic challenges against pancreatic carcinoma.

Observation:

This study reviewed ten cases of AIP in patients aged 26–56 years (six males).
Clinical presentations varied, including obstructive jaundice (6 cases), acute pancreatitis (2 cases), elevated serum amylase/lipase (1 case), and chronic abdominal pain with weight loss (1 case).
Imaging revealed a pancreatic mass in six patients, and endoscopic retrograde cholangiopancreatography (ERCP) showed ductal abnormalities in all four patients examined.

Findings:

Pathological examination of resected tissue from five patients confirmed inflammatory infiltration.
Five patients treated with corticosteroids demonstrated a positive clinical response.
AIP requires consideration in the differential diagnosis of pancreatic lesions, particularly pancreatic masses.

Implications:

Autoimmune pancreatitis should be considered in the differential diagnosis of pancreatic lesions, especially when malignancy is suspected.
Prompt diagnosis and treatment with steroids can lead to favorable outcomes.
Further research into the specific immunological mechanisms and long-term management of AIP is warranted.
