Summary
This case study highlights diffuse malignant pleural mesothelioma (DMPM) treatment. Multimodality therapy, including surgery and chemotherapy, improved survival and quality of life for this patient.
Area of Science:
- Oncology
- Thoracic Surgery
- Pulmonology
Background:
- Diffuse malignant pleural mesothelioma (DMPM) is a rare and aggressive cancer.
- Asbestos exposure is a primary risk factor for DMPM.
- Diagnosis often involves imaging, thoracoscopy, and histology.
Observation:
- A 68-year-old male presented with right-sided pleural effusion, dyspnea, and chest pain.
- Initial investigations (thoracentesis, CT scan) were inconclusive for malignancy.
- Videothoracoscopy revealed nodular lesions on the parietal pleura, diagnosed as epithelial DMPM with sarcomatoid areas.
Findings:
- The patient underwent Extrapleural Pleuropneumonectomy followed by Cisplatin and Pemetrexed chemotherapy.
- A disease-free period of one year was achieved.
- The patient survived for 31 months, succumbing to septic complications.
Implications:
- Single treatment modalities show limited efficacy for DMPM.
- Multimodality therapy offers improved survival and quality of life for DMPM patients.
- This case underscores the importance of a comprehensive treatment approach for DMPM.
