Background:

  • Desmoid tumors, also known as fibromas, are rare mesenchymal neoplasms.
  • Mesenteric desmoids typically arise in patients with Gardner's syndrome or a history of abdominal surgery.
  • Isolated mesenteric desmoids without these risk factors are exceptionally uncommon.

Observation:

  • A unique case of an isolated mesenteric desmoid tumor is presented.
  • The tumor originated from the mesentery of the appendix, an atypical location.
  • The desmoid presented as a twisted mass, a highly unusual morphology.

Findings:

  • Computed tomography (CT) and ultrasound (US) imaging characteristics of the twisted mesenteric desmoid are described.
  • The patient had no identifiable risk factors, such as Gardner's syndrome or prior abdominal surgery, for mesenteric fibrosis.
