Photochemical Electrocyclic Reactions: Stereochemistry01:26

Photochemical Electrocyclic Reactions: Stereochemistry

The absorption of UV–visible light by conjugated systems causes the promotion of an electron from the ground state to the excited state. Consequently, photochemical electrocyclic reactions proceed via the excited-state HOMO rather than the ground-state HOMO. Since the ground- and excited-state HOMOs have different symmetries, the stereochemical outcome of electrocyclic reactions depends on the mode of activation; i.e., thermal or photochemical.

Allergic Drug Reactions01:27

Allergic Drug Reactions

Allergic reactions related to drugs are hypersensitivity responses driven by the immune system and bear no connection to the drug's therapeutic action. While drugs in isolation do not trigger an immune response, they can interact with endogenous proteins to form antigens. These antigens stimulate lymphocytes to produce antibodies. IgE-type antibodies attach themselves to mast cells. Upon subsequent exposure to the same stimulus, the antigen-antibody interaction is initiated, unleashing numerous...
Thermal and Photochemical Electrocyclic Reactions: Overview01:26

Thermal and Photochemical Electrocyclic Reactions: Overview

Electrocyclic reactions are reversible reactions. They involve an intramolecular cyclization or ring-opening of a conjugated polyene. Shown below are two examples of electrocyclic reactions. In the first reaction, the formation of the cyclic product is favored. In contrast, in the second reaction, ring-opening is favored due to the high ring strain associated with cyclobutene formation.

Cycloaddition Reactions: MO Requirements for Photochemical Activation

Some cycloaddition reactions are activated by heat, while others are initiated by light. For example, a [2 + 2] cycloaddition between two ethylene molecules occurs only in the presence of light. It is photochemically allowed but thermally forbidden.
Allergic Reactions: Anaphylaxis01:30

Allergic Reactions: Anaphylaxis

Anaphylaxis is a severe, life-threatening hypersensitivity reaction mediated by Immunoglobulin E (IgE) antibodies. When IgE binds to allergens, it triggers the release of mediators– histamine, leukotrienes, and prostaglandins from mast cells and basophils. These mediators cause vasodilation, edema, and inflammation, leading to various symptoms. The primary allergens causing anaphylaxis include food items (e.g., peanuts, shellfish), drugs (e.g., penicillin, asparaginase, corticotropin, heparin),...

Summary
This summary is machine-generated.

Octocrylene is a significant allergen causing contact dermatitis in children and photoallergic reactions in adults, often linked to ketoprofen photoallergy. Patients with ketoprofen photoallergy frequently react to octocrylene, requiring avoidance of certain sunscreens.

Area of Science:

  • Dermatology
  • Allergology
  • Photodermatology

Background:

  • Octocrylene is an emerging photoallergen.
  • This study investigates 50 cases of photoallergic contact dermatitis linked to octocrylene.
  • An unexpected association between octocrylene photoallergy and prior ketoprofen photoallergy in adults is highlighted.

Purpose of the Study:

  • To report and discuss cases of photoallergic contact dermatitis caused by octocrylene.
  • To identify the prevalence of octocrylene allergy in different patient groups.
  • To explore the link between ketoprofen photoallergy and octocrylene sensitization.

Main Methods:

  • Patients were categorized into three groups: children (group A), adults with sunscreen photoallergy (group B), and adults with ketoprofen photoallergy tested for octocrylene (group C).
  • Patch testing and photopatch testing were conducted to assess reactions to octocrylene and other allergens.
  • Clinical histories of photoallergy and reactions to specific allergens were analyzed.

Main Results:

  • A high percentage of patients across all groups showed positive reactions to octocrylene.
  • Photopatch test reactions to octocrylene were common, particularly in adults with a history of ketoprofen photoallergy.
  • Many patients with ketoprofen photoallergy also reacted to other common allergens like fragrance components.

Conclusions:

  • Octocrylene is a potent allergen causing contact dermatitis in children and photoallergic contact dermatitis in adults.
  • A significant association exists between ketoprofen photoallergy and positive photopatch test reactions to octocrylene.
  • Patients with ketoprofen photoallergy should be informed about sunscreens containing octocrylene, benzophenone-3, or fragrances.
