Background:

  • Concerns exist regarding the safety and quality of percutaneous coronary intervention (PCI) performed without onsite cardiac surgery.
  • This study presents the outcomes of the first Danish center to offer PCI without onsite cardiac surgery.

Purpose of the Study:

  • To evaluate the safety, efficacy, and cost-effectiveness of performing PCI without onsite cardiac surgery.
  • To assess the impact on treatment latency, mortality, major adverse cardiovascular events, symptom relief, and quality of life.

Main Methods:

  • A total of 245 patients underwent 254 elective PCIs over a six-month period.
  • Outcomes measured included treatment latency, healthcare costs, mortality, MACE, symptom relief, and quality of life at six-month follow-up.

Main Results:

  • Median treatment latency significantly reduced for both stable and unstable disease patients (p < 0.05).
  • Healthcare costs decreased by 4,246,652 DKK.
  • Six-month mortality was 0.0% (stable) and 1.4% (unstable), lower than predicted scores.
  • No patients required emergency bypass surgery; angina improved in 92% and quality of life in 73%.

Conclusions:

  • Performing PCI without onsite cardiac surgery is safe and feasible.
  • This model of care enhances quality of care and significantly reduces healthcare costs.
  • Patient-reported outcomes, including symptom relief and quality of life, were excellent.
