Summary

In primary hyperparathyroidism (PHPT), high osteocalcin (OC) levels are linked to insulin resistance. While OC predicts insulin sensitivity in PHPT, its normalization post-surgery without improved insulin sensitivity suggests a minor role.

Background:

  • Osteocalcin (OC) is implicated in regulating insulin sensitivity.
  • Primary hyperparathyroidism (PHPT) is associated with elevated OC and insulin resistance.

Purpose of the Study:

  • To investigate the relationship between osteocalcin levels and insulin resistance markers in PHPT patients.
  • To assess if this relationship persists after parathyroidectomy (PTX).

Main Methods:

  • Serum OC, fasting insulin, and glucose levels were measured in 219 PHPT patients.
  • Insulin sensitivity was estimated using homeostatic model assessment (HOMA2-S%).
  • Parameters were re-evaluated in 45 patients post-PTX.

Main Results:

  • PHPT patients exhibited high OC levels, which were lower in diabetics compared to normal or impaired glucose tolerance groups.
  • OC showed a negative association with fasting glucose and a positive association with HOMA2-S%.
  • Post-PTX, OC levels decreased, but HOMA2-S% remained unchanged.

Conclusions:

  • Osteocalcin is a predictor of insulin sensitivity in PHPT.
  • The lack of change in insulin sensitivity after OC normalization post-PTX suggests OC may not be a major driver of insulin resistance in PHPT.
