  • Four experienced breast radiologists classified mammograms as missed or true.
  • Mammographic features were analyzed using a modified Breast Imaging Reporting and Data System (BI-RADS).

    • 30% of interval cancers and 21% of screen-detected cancers were classified as missed.
    • Calcifications were more frequent in missed cancers (34%) compared to screen-detected cancers (21%) on prior mammograms.
    • Masses were more common in screen-detected cancers (68%) than missed cancers (54%).

    Conclusions:

    • Calcifications pose a challenge in interpreting screening mammograms.
    • Reviewing both interval and screen-detected cancers is essential for education and improving detection rates.
    • Systematic reviews and adherence to guidelines can potentially reduce missed cancers in mammographic screening programs.
