A modified laparoscopic Vecchietti vaginoplasty using endovaginal ultrasound enhances safety and avoids serious injuries. The procedure results in a neovagina with good sexual function and high patient satisfaction.

To detail a minor modification of the laparoscopic Vecchietti vaginoplasty.
To assess the safety and efficacy of this modified technique.

The modification involved using an endovaginal ultrasound transducer to visualize the vesico-rectal space.

Sexual function was assessed using the Female Sexual Function Index (FSFI) and compared to 25 age-matched controls.

Main Results:

The endovaginal ultrasound modification improved surgical safety, preventing bladder and rectal injuries.
Overall FSFI scores and most sexual function domains (desire, arousal, orgasm, satisfaction) were comparable to the control group.
Neovagina patients reported slightly less lubrication and increased pain during intercourse.

Conclusions:

The modified laparoscopic Vecchietti operation is a safe and effective method for neovagina creation.
The procedure yields a neovagina that supports a good quality of sexual life with high patient satisfaction.
Endovaginal ultrasound guidance is a valuable addition to enhance surgical precision and safety.
