Summary

Small, hypopigmented papules on the trunk are a scarring process secondary to acne vulgaris. These papular acne scars are distinct from other previously described conditions.

Background:

  • A cutaneous eruption of small, asymptomatic, hypopigmented, follicular papules on the upper trunk was observed.
  • Acne vulgaris is a common skin condition with various sequelae.

Purpose of the Study:

  • To investigate the nature of these papular lesions.
  • To determine their association with acne vulgaris.
  • To propose a classification for these lesions.

Main Methods:

  • Clinical examination of 133 patients.
  • Dermatopathological analysis of biopsy specimens from five patients.

Main Results:

  • Papules were present in 57% of patients with a history of acne vulgaris versus 9% without.
  • Biopsies showed attenuated elastic and collagen fibers in perifollicular/parafollicular lesions, consistent with scarring.
  • The observed changes are similar to perifollicular elastolysis and papular elastorrhexis.

Conclusions:

  • The papules represent a scarring process secondary to acne vulgaris.
  • The term "papular acne scars" is proposed for these lesions.
  • These scars are closely related to or identical with previously described conditions.
