The thoracic or rib cage forms the body's thorax (chest) portion. Its primary function in the body is to protect vital organs in the thoracic cavity, such as the heart and the lungs. It consists of 12 pairs of ribs with their costal cartilages and the sternum. The ribs are anchored posteriorly to the 12 thoracic vertebrae (T1-T12).
The sternum is the elongated bony structure on the anterior side of the thoracic cage. It consists of three parts: the manubrium, the body, and the xiphoid process.

Once the aorta traverses the diaphragmatic plane at the aortic hiatus, it is known as the abdominal aorta.   • Computed tomography revealed a complex cystic formation with capsular visualization, compressing adjacent structures including the pancreas and left adrenal gland.
  • The mass extended through the diaphragm into the left thoracic cavity, indicating a thoracoabdominal presentation.

Findings:

  • Initial intervention involved cyst drainage and fenestration.
  • A subsequent radical surgical resection included splenectomy, left adrenalectomy, left nephrectomy, and removal of the tumor from the thoracic space.
  • Histopathological examination confirmed a mature teratoma, specifically a dermoid cyst, with skin-like morphology.

Implications:

  • This case underscores the importance of considering rare presentations of teratomas, even when benign.
  • The management of large, thoracoabdominal teratomas necessitates complex surgical strategies due to their extensive location and potential for organ displacement.
  • Complete surgical excision is crucial for definitive treatment, even in the absence of malignant transformation, to alleviate symptoms and prevent complications.
