Sleep apnoea syndrome (SAS) in chronic heart failure (CHF) is linked to higher left ventricular pressures. Central sleep apnoea (CSA) patients show a worse functional state than obstructive sleep apnoea (OSA) patients.
Area of Science:
- Cardiology
- Sleep Medicine
- Pulmonology
Background:
- Sleep apnoea syndrome (SAS) is common in chronic heart failure (CHF), but its origins are not fully understood.
- Understanding associated factors and differentiating between central sleep apnoea (CSA) and obstructive sleep apnoea (OSA) in CHF patients is crucial for management.
Purpose of the Study:
- To identify factors associated with SAS in patients with heart failure (HF).
- To compare the characteristics of central sleep apnoea (CSA) and obstructive sleep apnoea (OSA) within the heart failure population.
Main Methods:
- A cross-sectional, retrospective study involving 30 patients with stable heart failure on optimal medical therapy.
- Data collection included polygraphy, echocardiography, and cardiopulmonary exercise testing.
Main Results:
- SAS was present in 60% of patients (33.3% CSA, 26.7% OSA).
- Higher body mass index, blood pressure, and left ventricular pressures (E/Ea ratio) were associated with SAS.
- CSA patients exhibited higher New York Heart Association class and lower peak VO2 compared to OSA patients.
Conclusions:
- Elevated left ventricular pressures, indicated by the E/Ea ratio, are significantly associated with SAS in heart failure patients.
- Central sleep apnoea (CSA) appears to be linked to a poorer functional status in heart failure compared to obstructive sleep apnoea (OSA).
