Summary

Early statin therapy after acute coronary syndromes (ACS) did not reduce death or heart attack within four months. However, it significantly lowered unstable angina occurrences, with some risk of muscle toxicity, particularly with high-dose simvastatin.

Background:

  • Acute coronary syndromes (ACS) pose a high risk for recurrent events and mortality.
  • The short-term impact of early statin treatment in ACS patients remains unclear.

Purpose of the Study:

  • To evaluate the benefits and harms of early statin administration in ACS patients.
  • Analysis based on randomized controlled trials (RCTs).

Main Methods:

  • Searched CENTRAL, MEDLINE, EMBASE, CINAHL up to February 2010.
  • Included RCTs comparing statins vs. placebo/usual care initiated within 14 days of ACS, with ≥30 days follow-up.
  • Pooled data using random effects models to calculate risk ratios (RRs).

Main Results:

  • 18 RCTs (14,303 patients) analyzed.
  • No significant reduction in death, non-fatal myocardial infarction (MI), or stroke at 1 or 4 months.
  • Significant reduction in unstable angina at 4 months (RR 0.76).
  • Nine cases of myopathy (0.13%) with statins vs. one (0.015%) in controls; serious toxicity linked to simvastatin 80 mg.

Conclusions:

  • Early statin initiation within 14 days of ACS does not reduce mortality, MI, or stroke up to 4 months.
  • Early statin therapy effectively reduces unstable angina occurrence at 4 months post-ACS.
  • Potential for myopathy exists, especially with high-dose simvastatin.
