Oogenesis02:07

Oogenesis

In human women, oogenesis produces one mature egg cell or ovum for every precursor cell that enters meiosis. This process differs in two unique ways from the equivalent procedure of spermatogenesis in males. First, meiotic divisions during oogenesis are asymmetric, meaning that a large oocyte (containing most of the cytoplasm) and minor polar body are produced as a result of meiosis I, and again following meiosis II. Since only oocytes will go on to form embryos if fertilized, this unequal...

Area of Science:

  • Gynecology
  • Reproductive Medicine
  • Surgical Pathology

Background:

  • Bilateral salpingectomy is a common sterilization procedure.
  • Tubal endometriosis is a rare complication, often asymptomatic or misdiagnosed.
  • Endometrial tissue can implant and proliferate in residual fallopian tube segments.

Observation:

  • A 48-year-old woman presented with lower abdominal pain and menorrhagia.
  • She had a history of bilateral salpingectomy via Pomeroy technique four years prior.
  • Ovarian cyst was noted on ultrasonography, leading to hysterectomy and bilateral salpingo-oophorectomy.

Findings:

  • Histopathological examination revealed endometrial tissue replacing the mucosa of the tubectomy stump.
  • This confirmed the diagnosis of tubal endometriosis.
  • The condition was associated with an ovarian cyst.

Implications:

  • Tubal endometriosis is an under-recognized cause of gynecological symptoms post-salpingectomy.
  • Thorough sampling of fallopian tube stumps during histopathology is crucial for diagnosis.
  • Increased awareness may improve diagnosis and management of this rare entity.
