Thyroid eye disease can cause strabismus after cataract surgery, even without prior symptoms. This condition should be considered in post-operative diplopia diagnosis.

Background:

  • Strabismus following cataract surgery is often attributed to operative trauma.
  • A definitive etiology for motility disturbances is not always found through standard examinations.

Observation:

  • A study of 58 patients with post-cataract surgery strabismus identified eight (14%) with previously unsuspected thyroid eye disease.
  • Three patients had a history of thyroid disease, but no prior diplopia.
  • Five patients had no prior diplopia, and three of these had normal thyroid function tests.

Findings:

  • Absence of pre-operative diplopia may be due to improved vision post-surgery, intraoperative aggravation of subclinical thyroid eye disease, or coincidental onset.
