Purpose of the Study:

  • To evaluate regional LV function after SVR using overlapping left ventriculoplasty (OLVP).
  • To assess the effects of SVR and papillary muscle approximation (PMA) on regional LV wall function in heart failure patients.

Main Methods:

  • Quantitative gated myocardial perfusion SPECT (QGS) was used to assess regional LV function.
  • Forty-one heart failure patients (ischemic cardiomyopathy [ICM] and non-ICM [NICM]) underwent SVR and/or PMA.
  • Regional wall thickening was compared pre- and post-operatively across four surgical/etiology groups.

Main Results:

  • NYHA functional classes improved in all groups post-operation.
  • MR grade improved in three groups, excluding SVR for ICM.
  • LV basal wall thickening improved significantly after SVR (ICM and NICM) and PMA (ICM), but not after PMA in NICM patients.

Conclusions:

  • OLVP effectively improves NYHA functional class and basal LV regional wall function.
  • Lone PMA procedures do not demonstrate significant improvement in regional LV wall function at any level.
