Aneurysmal bone cysts are rare, benign bone tumors. This case highlights a rare occipital bone presentation in a pediatric patient, emphasizing surgical treatment and radiotherapy for incomplete excisions.

- Aneurysmal bone cysts (ABCs) are rare, benign, expansile, osteolytic bone lesions.

- Histopathological examination confirmed the diagnosis of an aneurysmal bone cyst.
- The lesion demonstrated characteristic features of ABCs on imaging and microscopy.
Implications:
- Surgical excision is the primary treatment for symptomatic or growing ABCs.
- Radiotherapy may be considered as an adjunct treatment for incompletely excised lesions or in cases where surgery is not feasible.
