Inflammation and remodelling patterns in early stage chronic rhinosinusitis

In early stage chronic rhinosinusitis without nasal polyps (CRSsNP), transforming growth factor-beta 1 (TGF-beta 1) protein is elevated in sinuses, suggesting it drives early disease. Other inflammatory markers were not significantly different, highlighting TGF-beta 1 as a key therapeutic target.

- Collagen deposition was analyzed as an indicator of TGF-beta signaling.

- TGF-beta 1 protein was significantly increased in maxillary sinuses, uncinate process, and anterior/posterior ethmoid compared to turbinates.
- Collagen deposition was higher in the maxillary sinus versus inferior turbinates.
- mRNA for TGF-beta receptors, Th1 markers, pro-inflammatory cytokines, and MPO showed no significant location-based differences.
- TGF-beta 1 mRNA was higher in CRSsNP inferior turbinates versus controls, but other markers did not differ.
Conclusions:
- Early-stage CRSsNP shows elevated TGF-beta 1 protein in paranasal sinuses, but not in turbinates.
- Pro-inflammatory, neutrophilic, and Th1 markers did not differ significantly across locations or between patients and controls.
- These findings implicate TGF-beta 1 in the initiation of CRSsNP and suggest it as a potential therapeutic target.
