Computed tomography (CT) is useful for detecting centriacinar emphysema (CAE) and its severity. While CT shows high reliability, it has limitations in diagnosing mild CAE cases.

Area of Science:

  • Pulmonology
  • Radiology
  • Pathology

Background:

  • Centriacinar emphysema (CAE) is traditionally diagnosed pathologically.
  • Computed tomography (CT) offers detailed anatomic visualization, making it a potential diagnostic tool for emphysema.

Purpose of the Study:

  • To assess the reliability and detection capability of CT in diagnosing CAE.
  • To evaluate the accuracy of CT in determining the severity of CAE through a CT-pathologic correlative study.

Main Methods:

  • 20 lung lobes with CAE were analyzed using CT and pathological examination.
  • CT scans were correlated with pathological findings using a point-counting method for grading CAE severity.
  • Three physicians visually assessed CT scans for low-attenuation areas.

Related Experiment Videos

Main Results:

  • A strong correlation (r = 0.94) was observed between CT and pathological scores for CAE.
  • CT detection rates for CAE were 74% (mild), 91% (moderate), and 100% (severe).
  • CT could detect emphysematous spaces with diameters as small as 3-4 mm.

Conclusions:

  • CT is highly useful for assessing the extent and severity of CAE.
  • CT has limitations in diagnosing mild cases of CAE.
  • Visual scoring of low-attenuation areas on CT is clinically valuable for quantitative CAE evaluation.
