Flat panel CT (fpCT) offers artifact-free imaging of the middle and inner ear after stapesplasty, correlating hearing improvement with prosthesis position. This technique provides detailed post-operative evaluation with lower radiation than standard CT.

Area of Science:

  • Otolaryngology
  • Medical Imaging
  • Biomedical Engineering

Background:

  • Post-stapesplasty evaluation requires detailed anatomical information, especially for unsuccessful air-bone gap (ABG) closure.
  • Standard computed tomography (CT) is limited by metal artifacts and high radiation doses, particularly after middle ear implant surgery.

Purpose of the Study:

  • To validate the accuracy of flat panel CT (fpCT) for imaging the middle and inner ear.
  • To assess the correlation between post-operative hearing results and prosthesis position using fpCT.
  • To compare fpCT with standard CT for evaluating stapesplasty outcomes.

Main Methods:

  • fpCT was performed on three temporal bone specimens with experimental stapes prostheses.
  • Post-operative fpCT was conducted on 28 patients with various stapes prostheses.

  • Hearing outcomes were compared with determined prosthesis positions.
  • Main Results:

    • fpCT demonstrated a statistically significant correlation between hearing improvement and prosthesis position in all cases.
    • The technique provided detailed post-operative imaging of implant placement in both specimens and patients.
    • fpCT showed higher isovolumetric resolution and lower radiation dose compared to standard CT.

    Conclusions:

    • fpCT enables immediate, artifact-free evaluation of stapesplasty surgical results.
    • This imaging modality aids otosurgeons in designing precise prostheses and predicting post-operative hearing outcomes.
    • fpCT serves as an adequate tool for immediate post-operative quality control and appropriate management of prosthesis-related complications.
