Bones of the Upper Limb: Humerus

The upper limb consists of the arm, forearm, wrist, and hand bones. The humerus is the single bone of the upper arm region. Purpose of the Study:

  • To report on an Objective Structured Assessment of Technical Skills (OSATS) format for evaluating orthopedic surgery residents' technical skills.
  • To assess residents' proficiency in three common upper extremity surgeries: trigger finger release (TFR), open carpal tunnel release, and distal radius fracture fixation.
  • To determine the validity and educational value of a multi-station OSATS in hand surgery training.

Main Methods:

  • Twenty-seven residents across postgraduate years 2-5 participated.
  • Residents performed simulated surgeries on cadaveric specimens at three distinct stations.
  • Performance was evaluated using procedure-specific checklists, global rating scales, and pass/fail assessments by a board-certified hand surgeon.

Main Results:

  • Construct validity was demonstrated through correlations between training year and assessment scores for TFR and carpal tunnel release.
  • Criterion validity was shown by correlating different assessment methods for all three procedures.
  • Surgical performance did not correlate with the time taken to complete the procedures; residents found the OSATS format highly educational.

Conclusions:

  • A surgeon's skill in one procedure (e.g., TFR) does not necessarily correlate with proficiency in another (e.g., carpal tunnel release).
  • A single assessment tool is insufficient for comprehensively evaluating residents' surgical abilities across varying complexities.
  • A combination of checklist, global rating, and pass/fail assessments is necessary to fully understand a resident's technical competence.
