Children with bilateral cochlear implants (CIs) often need different stimulation levels for each ear. Objective measures like electrically evoked compound action potential (ECAP) thresholds can help achieve balanced CI input.

  • Electrically evoked compound action potential (ECAP) and electrically evoked stapedius reflex (ESR) thresholds were recorded.
  • Main Results:

    • Stimulation levels were consistently higher for the first CI (CI-1) compared to the second CI (CI-2).
    • The dynamic range of stimulation was reduced for CI-2.
    • Differences in behavioral measures correlated significantly with differences in ECAP and ESR thresholds.

    Conclusions:

    • Children with bilateral CIs often require different stimulation levels for each implant.
    • Objective measures, particularly ECAP thresholds, show promise in predicting and achieving balanced bilateral CI stimulation.
    • Newer technology in the second CI may require lower stimulation levels but offers a reduced dynamic range.
