A rare yolk sac tumour was diagnosed in an elderly man. This elderly-onset testicular cancer presents differently than in younger patients, highlighting the need for specific diagnostic considerations.

- A radical orchiectomy was performed for the tumor.
- Histological examination revealed a malignant germ cell tumor of the yolk sac type.
Findings:
- The tumor showed rete testis invasion.
- No vascular or lymphatic invasion was observed.
- The tumor did not extend through the tunica albuginea.
- Postoperative computed tomography (CT) of the abdomen showed no signs of metastasis.
- Tumor markers remained normal after surgery.
Implications:
- Yolk sac tumors in the elderly are exceptionally rare.
- The clinical behavior of YSTs in the elderly may differ from that in younger populations.
- Accurate diagnosis is crucial for appropriate management and prognosis in elderly patients with testicular tumors.
