Purpose of the Study:

To evaluate the reliability, validity, and clinical utility of the Patient Health Questionnaire-9 (PHQ-9) and Patient Health Questionnaire-2 (PHQ-2) in stroke patients within a clinical nursing setting.
To compare the performance of PHQ-9 and PHQ-2 against the Geriatric Depression Scale.

Main Methods:

The PHQ-9 was administered by 43 nurses to 55 stroke patients (intracerebral hemorrhage or ischemic infarction) capable of adequate communication.

Evaluated metrics included interrater reliability, test-retest reliability, internal consistency, concurrent validity, diagnostic accuracy, and clinical utility.

Main Results:

PHQ-9 demonstrated good interrater reliability (ICC=0.98), test-retest reliability (ρ(Sp)=0.75), and internal consistency (Cronbach's α=0.79).
Concurrent validity was moderate for PHQ-9 (r=0.7) and acceptable for PHQ-2 (r=0.8).
Optimal cutoff points were PHQ-9=10 (sensitivity 100%, specificity 86%) and PHQ-2=2 (sensitivity 100%, specificity 77%) for major depression detection.

Conclusions:

The Patient Health Questionnaire (PHQ) is a brief, user-friendly instrument suitable for nursing practice.
The PHQ demonstrates good reliability, validity, and clinical utility for assessing depression in communicative stroke patients.
