A case of lethal midline granuloma is reported, which resulted from immunosuppressive therapy with corticoids over months because of pulmonary fibrosis. An autoimmune disease according to a cell mediated immune reaction (type IV--classification of Coombs and Gell) is discussed. Wegener's granulomatosis and a T-cell lymphoma were excluded. Continuation of this lymphoproliferative disease would eventually have resulted in a T-cell lymphoma.
