Peptide receptor radionuclide therapy (PRRT) is effective for neuroendocrine tumors. Tandem therapy with (90)Y/(177)Lu-DOTATATE shows improved survival and safety compared to single-isotope treatments.

Background:
- Neuroendocrine tumors (NETs) are a heterogeneous group of malignancies.

- Peptide receptor radionuclide therapy (PRRT) utilizes radiolabeled somatostatin analogs to target NET cells.
Purpose of the Study:
- To evaluate the efficacy and safety of PRRT in a Polish patient cohort.
- To compare outcomes between single-isotope and tandem PRRT regimens.
- To assess progression-free survival (PFS), overall survival (OS), and toxicity profiles.
Main Methods:
- Retrospective analysis of 358 patients treated with PRRT from April 2004 to December 2010.
- Therapies included (90)Y-DOTATATE, (177)Lu-DOTATATE, and tandem (90)Y/(177)Lu-DOTATATE.
- Patient outcomes and adverse events were monitored.
Main Results:
- (90)Y-DOTATATE therapy showed PFS/TTP of 17-44 months and OS of 22-34.2 months.
- Tandem (90)Y/(177)Lu-DOTATATE therapy resulted in longer PFS/TTP (24.2-28.3 months) and OS (49.8-52.8 months).
- PRRT was generally well-tolerated; grade 3/4 hematological and renal toxicity was primarily observed with (90)Y-DOTATATE.
Conclusions:
- PRRT is an effective and safe treatment for metastatic or inoperable NETs.
- Tandem (90)Y/(177)Lu-DOTATATE therapy demonstrated superior overall survival compared to single-isotope treatments.
- Hematological toxicity was rare, and significant renal toxicity was associated mainly with (90)Y-DOTATATE.
