Summary

Dens invaginations are rare dental defects. This study reports a unique case of bilateral Type I dens invaginations in mandibular premolars, presenting with periapical pathology despite healthy tooth appearance.

Area of Science:

  • Dentistry
  • Oral Pathology
  • Developmental Biology

Background:

  • Dens invaginations are uncommon developmental anomalies of teeth.
  • They predominantly affect maxillary lateral incisors, with rare occurrences in mandibular teeth.

Purpose of the Study:

  • To report a rare case of bilateral Type I dens invaginations.
  • To highlight the occurrence in mandibular first premolars.
  • To document associated periapical pathology in seemingly healthy teeth.

Main Methods:

  • Clinical examination of affected teeth.
  • Radiographic analysis (e.g., periapical radiographs, CBCT).
  • Histopathological examination (if available).

Lateral Molar Approach-Driven Transoral Endoscopic Procedure for Benign Infratemporal Fossa Tumor Resection

Published on: August 15, 2025

  • Bilateral Type I dens invaginations were identified in the first mandibular premolars.
  • The affected teeth, despite their invaginated structure, appeared clinically healthy.
  • Periapical pathology was detected adjacent to the invaginated teeth.

Conclusions:

  • Bilateral dens invaginations of the first mandibular premolars are exceptionally rare.
  • Dens invaginations, even when seemingly asymptomatic, can be associated with periapical pathology.
  • This case underscores the importance of thorough radiographic evaluation for detecting subtle dental anomalies and their sequelae.
