Background:

  • Hydrogel intraocular lenses (IOLs) are widely used in cataract surgery.
  • Calcification of IOLs can lead to visual impairment.

Observation:

  • A 48-year-old male presented with decreased visual acuity due to a milky white, opacified hydrophilic acrylic IOL.
  • Explantation revealed diffuse, fine, basophilic granular deposits within the IOL optic, confirmed as high calcium content via von Kossa staining.
  • Microscopic examination showed these deposits were separated from the surface by a clear zone, and Gram Tworts' staining ruled out microbial presence.

Findings:

  • The explanted hydrogel IOL exhibited significant calcification, characterized by intra-lens calcium deposits.
  • Pathological analysis confirmed the presence of calcium without evidence of microorganisms.
