Abstract:
Medication reconciliation has proved difficult to implement in practice. Medication reconciliation is a requirement in the Danish Healthcare Quality Programme. The purpose is to ensure that the patient receives the correct medicine. The guidelines for medication reconciliation in the Danish Quality Model are clearly reflected in the actual guidelines for medication reconciliation at hospitals in Denmark. However, it does not match the complexity of the clinical reality. If the distance between the clinical reality and the actual planned action becomes too large, problems might occur with the implementation of planned actions.
