Kimura disease involving a caruncle.

Background:

• Kimura disease is a rare, chronic inflammatory condition of unknown etiology, typically affecting the salivary glands and lymph nodes.
• Ocular involvement is uncommon, and localization to the ocular caruncle has not been previously reported.

Observation:

• A 35-year-old woman presented with a painless, slow-growing nodule in the left eye caruncle.
• Initial investigations including VDRL, ESR, ACE levels, and chest X-ray were normal.
• Ocular examination revealed normal visual acuity and ocular movements.

Findings:

• Excisional biopsy of the caruncular nodule revealed follicular hyperplasia with perifollicular fibrosis, eosinophil infiltrate, and capillary proliferation.
• Histopathology confirmed Kimura disease, characterized by normal-appearing endothelial cells lining the proliferating capillaries.

Implications:

• This is the first documented case of Kimura disease localized to the ocular caruncle.
• Kimura disease should be included in the differential diagnosis for caruncular mass lesions presenting in clinical practice.
• Early recognition and diagnosis are crucial for appropriate management and to prevent potential complications.
