Summary

This case study shows a successful treatment for erectile dysfunction using intracavernous alprostadil injections followed by tadalafil monotherapy. This combination therapy effectively improved erectile function and reduced patient distress.

Background:

Erectile dysfunction (ED) significantly impacts quality of life, often leading to psychological distress.
Sildenafil is a common first-line treatment for ED, but efficacy varies among patients.

Observation:

A 56-year-old male patient with a 12-year history of ED experienced persistent distress and depression.
Initial treatment with sildenafil was ineffective for improving erections.

Findings:

Intracavernous injection of alprostadil provided a moderate erection, enabling patient self-administration.
Subsequent treatment with tadalafil monotherapy proved successful in achieving sufficient erections.

Implications:

Sequential therapy with intracavernous alprostadil and oral tadalafil can be an effective strategy for managing refractory erectile dysfunction.
This approach offers a viable alternative for patients unresponsive to standard oral treatments.
Successful ED management can significantly improve psychological well-being and overall quality of life.
