Background:

A 70-year-old male with a history of prostate adenocarcinoma with metaplasia experienced refractory urinary retention.
The patient had previously undergone transurethral resection of prostate (TURP) and bilateral orchidectomy.

Observation:

Following initial TURP and orchidectomy, the patient presented again with urinary retention and similar histopathology findings.
A subsequent bone scan revealed solitary metastasis to the second rib, with MRI and CT confirming tumor regrowth.

Findings:

The resected prostatic tissue consistently showed differentiated adenocarcinoma with cartilaginous and osseous metaplasia.
Despite palliative radiation alleviating urinary symptoms, the patient opted for conservative management over radical cystoprostatectomy.

Implications:

This case underscores the aggressive nature of prostate adenocarcinoma with metaplasia and its potential for metastasis.
It emphasizes the importance of vigilant monitoring and tailored management strategies for recurrent or metastatic prostate cancer, even in palliative settings.
