  • The patient had previously undergone transurethral resection of prostate (TURP) and bilateral orchidectomy.

  • Following initial TURP and orchidectomy, the patient presented again with urinary retention and similar histopathology findings.
  • A subsequent bone scan revealed solitary metastasis to the second rib, with MRI and CT confirming tumor regrowth.

Findings:

  • The resected prostatic tissue consistently showed differentiated adenocarcinoma with cartilaginous and osseous metaplasia.
  • Despite palliative radiation alleviating urinary symptoms, the patient opted for conservative management over radical cystoprostatectomy.

Implications:

  • This case underscores the aggressive nature of prostate adenocarcinoma with metaplasia and its potential for metastasis.
  • It emphasizes the importance of vigilant monitoring and tailored management strategies for recurrent or metastatic prostate cancer, even in palliative settings.
