Summary

Revision endoscopic sinus surgery (RESS) objectively improved olfactory function in about half of patients with smell disturbance. This provides a valuable treatment option for chronic rhinosinusitis patients with poor olfactory prognoses.

Background:

  • Smell disturbance significantly impacts quality of life.
  • Endoscopic sinus surgery (ESS) can improve olfaction in chronic rhinosinusitis (CRS) patients.
  • Objective data on olfactory improvement after revision ESS (RESS) is limited.

Purpose of the Study:

  • To objectively assess olfactory function improvement after RESS.
  • To evaluate the efficacy of RESS in patients with smell disturbance.

Main Methods:

  • Olfactory function was measured using the University of Pennsylvania Smell Identification Test (UPSIT-TC).
  • Patients were assessed preoperatively and postoperatively at two time points (6-12 months and 12-24 months).
  • Improvement was defined as a shift from anosmia to hyposmia/normosmia or hyposmia to normosmia.

Main Results:

  • 32 patients with preoperative smell disturbance were included.
  • Mean Smell Identification Test (SIT) scores improved from 13.3 (preoperative) to 18.6 (6-12 months) and 20.4 (12-24 months).
  • Improvement rates were 44.8% and 47.8% at 6-12 and 12-24 months, respectively; nasal polyps and mucosal swelling correlated with better outcomes.

Conclusions:

  • RESS provides objective olfactory improvement in approximately 50% of patients.
  • RESS is a viable treatment option for smell dysfunction in CRS patients with limited alternatives.
  • The study offers objective evidence supporting RESS for olfactory restoration.
