Background:

Syphilis is a systemic infection with diverse ocular manifestations.
Acute zonal occult outer retinopathy (AZOOR) is a condition affecting the outer retinal layers.

Observation:

Two patients presented with clinical features suggestive of AZOOR complex diseases.
Initial presentations mimicked atypical outer retinopathy.

Findings:

Laboratory tests confirmed syphilis as the underlying cause in both cases.
Intravenous penicillin therapy resulted in significant improvement in visual acuity and symptoms.
Ancillary testing also showed positive responses to treatment.

Implications:

This case series highlights a novel presentation of ocular syphilis mimicking AZOOR.
Physicians should consider syphilis in the differential diagnosis of inflammatory outer retinopathies.
Prompt diagnosis and treatment of neurosyphilis are crucial for visual preservation.
