Efficacy and Safety of Rituximab in Biologic-Naive Patients with Rheumatoid Arthritis vs Anti-Tnf Therapy Failure

Rituximab (RTX) effectively treats rheumatoid arthritis (RA) in both biologic-naïve and anti-TNFα-experienced patients. However, remission is achieved faster in biologic-naïve individuals, who also experienced fewer severe infections.

- Anti-tumor necrosis factor-alpha (anti-TNFα) therapies are common RA treatments, but some patients are naïve or inadequate responders.
- Anti-CD20 therapy, such as rituximab (RTX), offers an alternative treatment option for RA.
Purpose of the Study:
- To compare the efficacy of rituximab (RTX) in rheumatoid arthritis (RA) patients.
- To evaluate RTX in anti-TNFα-naïve patients (Group 1) versus inadequate responders to anti-TNFα therapy (Group 2).

- Patients were categorized into anti-TNFα-naïve (n=13) or anti-TNFα-experienced (n=12) groups.
- Efficacy assessed by Disease Activity Score 28 (DAS28), acute phase reactants, and Health Assessment Questionnaire (HAQ) over a mean 6-month follow-up.

- Both groups showed clinical improvement and decreased acute phase reactants.
- Remission (DAS28) was achieved faster in the biologic-naïve group (Group 1) compared to the anti-TNFα-experienced group (Group 2).
- Severe herpes virus infections were more frequent in Group 2.
Conclusions:
- Rituximab is effective in both RA patient populations, achieving clinical remission.
- Disease duration and higher HAQ scores in the anti-TNFα-experienced group contributed to delayed remission.
- Biologic-naïve patients experienced faster remission and potentially fewer severe infections with RTX treatment.
