Proximal tibiofibular joint dislocation is a rare knee injury. Open reduction internal fixation with screw transfixation offers good results and a fast return to sports.

- Initial manual reduction under anesthesia failed.
Findings:
- Open reduction and internal fixation with screw transfixation of the proximal tibiofibular joint were performed.
- Following screw removal at six weeks, full weight-bearing and physical therapy were initiated.
- The patient returned to soccer activities within four months post-injury.
Implications:
- Proximal tibiofibular joint dislocation should be considered in the differential diagnosis of chronic lateral knee pain.
- Accurate diagnosis relies on technically correct X-rays, comparing the injured knee with the contralateral side.
- Surgical fixation provides favorable outcomes, enabling a rapid return to athletic pursuits.
